Wow, this was hard to find. Didn't remember what it was called. It was the Black Ops H2 from SEMA 2004:

It's a great looking truck. no doubt. That truck made me buy an SUT. But really there is nothing all that "special" about it. You can order the rims/ tires, door handles and have them black/ chromed. That brushguard is really the only thing unique that would have to be custom.
