4.5" backspacing is 4.5" backspacing regardless of the width of the rim...it means you've got 4.5" from the wheel mounting pad to the outside of the rim flange.
The reset of the wheel (in this case the other 4.5" goes out from the truck). If we were talking about a 10" rim with a 4.5" backspace you'd have
5.5" on the outside and your tires would stick out a bit further from your fenders.

EDIT: and, yeah, I like those rims too. If I'd have found them a little earlier that's what I'd have went with, but I already had my 18" tires in the garage waiting on my 18" ATX Mojave Teflons ...then they went out of production and couldn't be found anywhere. Unfortunately for me they don't make the Black Rocks in 18s; so it was Rockstars for me.
