Dont take this the wrong way...I'd be pissed if I was planning on a project and didn't know what else truely needed to be done to make it work. You have a low post count so I have no idea what your experience or knowledge is on this.
I'd search out on the various Hummer forums for other 12" lift owners and see if you can find one and call'em up directly. Speak w/ someone that has first hand experience as a non-show truck, how it handles and the maintenance required.
Wheels, I would not go over 20" unless you want to pay up the ass for rubber.
Lift that size, I wouldn't even attempt to drive it w/ out putting on ALL the upgraded steering components including solid centerlink. Plus an AGR Performance powersteering pump, stocker isn't even going to think about turning those tires under 10mph. Probably have 3G in parts alone, not including install.
Now your going to have a gutless turd on your hands, that couldn't pull out in traffic if your life depended on it. Here comes 5.13 gears. W 40" tires you will be really close to stock, should actually have a little more gear than stock. Good quality Yukon gears, master install kit w/ timken bearings will run you another grand. Plus $1100-1800 installation.
Now you can turn the wheels and actually drive the truck, your only problem now is it wont stop. I put brembo 6 piston caliper brakes on all 4 corners of my rig. You are going to need those, wilwoods or some other big ass brake kit. $5000-9000 Mind you these brakes are designed for factory stock spindles which you dont have anymore. I had about 5 hours or grinding metal to get my brembos to bolt up and have adequate clearance.
Those are the big ball busters...
