I wouldn't say that my truck started that rough, but it seems that as the truck got broken in, the engine start has become more smooth.
Mine used to crank a bit longer than in other cars I've owned, but only like a second longer, then turn over; But this only lasted for like the first 2k miles and now it turns over as quickly as I'd expect. I guess it needed to break in and allow the piston rings to seat better in the motor.

It was just long enough for me to notice that it wasn't the same as other 'new' cars that I had owned.

I also get that exhaust smell on startup, but this is more outside. If I'm out of the truck moving stuff into it while it warms up, but it certainly isn't offensive. Just a slight plume when the motor first starts and is cold.
